The programme offers FREE self-management courses for people living with any long-term health condition or those who care for them, contracted by the ABHB and are quality-assured by Public Health Wales.
Since 2002, the EPP has helped people improve their quality of life by learning different skills to better manage their condition on a daily basis, making them less dependent on health and social care programmes. The aim is to give participants the confidence to take responsibility for their own care, whilst also encouraging them to work in partnership with professionals.
There are a mixture of Introduction to Self-Management (one 3 hour-session) courses and our flagship six-week Health and Well Being courses. All are led by Public Health Wales–trained tutors who themselves live with a long-term health condition or who care for someone living with one.
The six-week course is 2 ½ hours per week for six weeks (the same day and time each week). There are 10-16 participants with a variety of conditions. Sessions cover topics such as:
- Managing symptoms such as pain and tiredness
- Getting a good night’s sleep
- Dealing with anger, fear and frustration
- Coping with stress, depression and low self – image
- Eating healthily and exercise
- Learning relaxation techniques and taking regular exercise
- Improved communication with family, friends and health professionals
- Planning for the future and making decisions
